Tailoring a plan for supported self-management to the patient

Professor Pinnock explains how personalised action plans are key for the supported self-management individuals living with asthma or COPD.

All people with long-term conditions take day-to-day decisions about living with their condition. Therefore, supporting them to take appropriate decisions is an integral component of care.

Hilary Pinnock was a co-author of the 2014 meta-review of supported self-management in 14 LTCs, including asthma and COPD. A taxonomy (Practical Reviews in Self-Management Support, or PRISMS, 2016) that provides a picklist of strategies appropriate for personalising action plans came from this work. It follows that the applicability of the different components will vary according to the nature of the conditions.
