This project from Nesta and the Health Foundation aimed to illustrate what good person-centred care looks like. Realising the Value offers tools, recommendations and economic models to demonstrate how person- and community-centred approaches can improve health and well-being outcomes.
Resources offered by the programme include a list of ten actions to put people and communities at the heart of health and wellbeing. An economic modelling tool is also provided, to help commissioners evaluate the potential impact of investing in a person- and community-centred approach. Finally, behavioural factors and how the science of behaviour can help people to self-manage their health are discussed.
