Integrated population care

Providing population-focused care necessitates rethinking how healthcare services are structured. Instead of individual providers, teams between and across specialties will become increasingly important. Moreover, barriers between primary and secondary care must be dismantled. Here, our two highly experienced speakers discuss how inter-specialty working is key to managing an ever more complex patient population.
