6. When would you recommend we start combination inhalers with inhaled steroids? Does this depend on an FEV1 below 50%, or on the frequency of exacerbations? Is it important to also look at whether the FEV1 is reversed after bronchodilation by at least 400ml to diagnose co-existent asthma?
